Most hospitals do not know where they actually stand until a surveyor — or an incident — tells them. The Operational Readiness Index™ is built to close that gap before either of those moments arrives.

The ORI is a structured, evidence-based assessment administered by a D2EMC senior advisor. It is not a self-reported checklist. Every score is assigned based on documented, verifiable evidence — plans, procedures, training records, exercise files, and direct advisor review. The methodology was developed by David DiGregorio, drawing on 32 years of military medical and state-level hazmat command experience and his ongoing faculty work with the BIDMC/Harvard Medical School Disaster Medicine Fellowship.

The assessment evaluates your full preparedness posture across every major domain: written plans and regulatory documentation, CBRN and decontamination readiness, staff training history and competency records, exercise design and execution, leadership command structure and HICS integration, and regional coalition participation. You receive a composite score out of 100, a tier designation, domain-level subtotals, and a prioritized improvement matrix — all in a leadership-ready report delivered in 5–7 business days.

Facilities scoring 60 or above receive a tier-based ORI Seal for display on facility signage, websites, and coalition reporting.

ORI Performance Tiers 80–100 — Excellence: Exceptional posture. Positioned to support regional coalition partners. 60–79 — Operational: Core readiness criteria met. Targeted improvement opportunities identified. 40–59 — Developing: Foundational elements exist. Structured improvement plan recommended. 0–39 — Critical Gaps: Significant deficiencies. Immediate corrective action required.
